Hiatus Hernia – Diagnosis & Treatment

A hiatus hernia occurs when the upper part of the stomach pushes through the diaphragm into the chest cavity via an opening called the hiatus. The diaphragm normally acts as a barrier between the chest and abdomen, and this condition can disrupt normal digestion, often leading to symptoms similar to acid reflux or heartburn.


Causes of Hiatus Hernia

While the exact cause is unclear, factors contributing to a hiatus hernia include:

  • Increased abdominal pressure (obesity, pregnancy, heavy lifting)

  • Age-related weakening of the diaphragm

  • Chronic coughing or straining

  • Congenital structural abnormalities

People over 50 years of age, pregnant individuals, and those who are overweight are at higher risk.


Types of Hiatus Hernia

  • Sliding Hiatus Hernia – The most common type, where the stomach and lower esophagus slide in and out of the chest through the hiatus.

  • Paraesophageal (Rolling) Hernia – Less common but potentially more serious, where part of the stomach pushes into the chest beside the esophagus.


Symptoms

Common symptoms include:

  • Heartburn and acid reflux

  • Chest discomfort or pain

  • Swallowing difficulties

  • Belching or a feeling of fullness

  • Vomiting or regurgitation in severe cases


Diagnostic Evaluation

Diagnosis is typically confirmed through:

  • Upper GI Endoscopy

  • Barium Swallow Study

  • Esophageal pH monitoring

  • Manometry (esophageal pressure test)


Treatment Options

Conservative Management

Small and asymptomatic hernias may be managed with lifestyle adjustments and medications such as:

  • Antacids or acid-reducing drugs

  • Dietary changes

  • Weight loss and exercise

  • Avoiding late-night meals and elevating the head during sleep

Surgical Management: Laparoscopic Hiatus Hernia Repair

For larger or symptomatic hernias, laparoscopic surgery is the preferred option. During this procedure:

  • The stomach is repositioned into the abdominal cavity

  • The diaphragmatic opening (hiatus) is tightened

  • A fundoplication is performed, wrapping the upper part of the stomach around the esophagus to reinforce the valve and prevent reflux
